7. PLANNING & ZONING COMMISSION REPORTS

A. Request by the Village of Hoffman Estates (owner and applicant) for a Master Sign Plan in accordance with Section 9-3-8-M-l 3 of the Zoning Code for the property located at 5333 Prairie Stone Parkway (Arena property). 

Voting: 11 Ayes, 0 Nays

8. ADDITIONAL BUSINESS

A. Request Board approval of an Ordinance amending Section 8-3-22, Number of Licenses of Article 3, Alcoholic Liquors, of the Hoffman Estates Municipal Code (decrease in number of Class "A", Class "D" and Class "DD" Licenses, for Whi Chicago Northwest Management LLC d/b/a Chicago Marriott Northwest, 4800 Hoffman Boulevard; and increase in number of Class "A", Class "D" And Class "DD" Licenses, for Hoffman Estates Hotel Group Llc D/B/ A Chicago Marriott Northwest, 4800 Hoffman Boulevard, and increase in Class "A" License for Poplar 2706 CC., d/b/a Honeyberry Cafe, 2708 N. Sutton Road) 

B. Request Board approval of an Ordinance granting a Master Sign Plan under Section 9-3-8-M-13 of the Zoning Code for the property located at 5333 Prairie Stone Parkway (Arena property). 

C. Request Board authorization to award contract for 2020 concrete maintenance/restoration Program to AC Concrete Construction, Carpentersville, IL (low bid) at a unit price of $8.80 per square foot for sidewalk, $29.00 per lineal foot for curb replacements, and $72.00 per square yard for driveway apron replacements, in an amount not to exceed $95,000. 

D. Request Board authorization to award contract for 2020-2023 Janitorial Maintenance Services for Village Hall, Police Department, Susan Kenley- Rupnow Public Works Center and Fleet Services Facility to XXXX, XXXX, XX, for total contract monthly fee of $XXX, total annual contract amount not to exceed $XXX. 

E. Request Board authorization to:

1) waive formal bidding; and 

2) purchase one 2020 Bobcat E35 compact track excavator from Atlas Bobcat, Elk Grove Village, IL in an amount not to exceed $45,505 through the Sourcewell Cooperative Purchasing Program. 

F. Request Board authorization to extend the existing contract for the provision of Village nuisance wildlife control to Animal Trackers Wildlife Company, Hoffman Estates, IL, for a period of one year. 

G. Request Board approval to enter into the Illinois Ground Emergency Medical Transportation (GEMT) Medicaid reimbursement program through an intergovernmental agreement (IGA) with the Illinois Department of Healthcare and Family Services (HFS). 

H. Request Board authorization to purchase twenty (20) sets of structural firefighting clothing: ten (10) sets from MES-Illinois, Deer Creek, IL, (sole source vendor), in an amount not to exceed $27,000, and ten (10) sets from Air One Equipment Inc., South Elgin, IL, (sole source vendor), in an amount not to exceed $27,000, for a total request not to exceed $54,000. 

I. Request Board approval of the Community Development Block Grant (CDBG) Consolidated Plan 2020-2024 and Annual Action Plan 2020.

8. ADDITIONAL BUSINESS - Continued

J. Request Board authorization to enter into an Intergovernmental and Subrecipient Agreement for Coronavirus Relief Funds between the Village of Hoffman Estates and Cook County, IL. 

K. Request Board approval of a Second Amendment to Lease to the existing Communications Equipment Site Lease Agreement with Comcast of Illinois VI, LLC for cable TV equipment at 95 Aster Lane.
