-Primary cause: pneumonia, COVID-19

-Secondary cause: diabetes mellitus, chronic obstructive pulmonary disease, hypertension, hyperlipidemia

74 year-old white male

Location of death: 5205 N. Ludlam Ave., Chicago

Died from:

-Primary cause: pneumonia, COVID-19

-Secondary cause: chronic obstructive pulmonary disease, hypertension, diabetes mellitus
