a. Consideration of a request from the Elk Grove Park District to waive permit fees in the amount of $60 for catch basin repair at Lions Park located at 180 Lions Dr.

(It has been past practice of the Village Board to grant fee waivers for governmental and non-profit organizations.

(The Director of Community Development recommends approval.)

b. Consideration of a request from the Salvation Army, located at 1000 Pratt Blvd., to waive permit fees for parking lot maintenance in the amount of $99.

(It has been past practice of the Village Board to grant fee waivers for governmental and non-profit organizations.

(The Director of Community Development recommends approval.)

c. Consideration of a request from School District 54, to waive FY 2017/18 business license fees in the amount of S100.

(It has been past practice of the Village Board to grant fee waivers for governmental and non-profit organizations.

(The Director of Finance recommends approval.)

d. Consideration of a request from Lutheran Church of the Holy Spirit, located at 150 Lions Drive, to waive permit fees for sanitary sewer service repair in the amount of $210.

(It has been past practice of the Village Board to grant fee waivers for governmental and non-profit organizations. (The Director of Community Development recommends approval.)

e. Consideration of a request from Alexian Brothers Medical Center, 801 Gloucester Drive, to waive permit fees for sanitary sewer service repair in the amount of $311.

(It has been past practice of the Village Board to grant fee waivers for governmental and non-profit organizations. (The Director of Community Development recommends approval.)

f. Consideration of a request from John Moore Vending to waive FY 2017/18 business license fees (for the United States Post Office) in the amount of $900.

(It has been past practice of the Village Board to grant fee waivers for governmental and non-profit organizations. (The Director of Finance recommends approval.)

g. Consideration of a request from the Elk Grove Park District, 1000 Wellington Avenue, to waive permit fees for locker room renovations in the amount of $1,870.

(It has been past practice of the Village Board to grant fee waivers for governmental and non-profit organizations. (The Director of Community Development recommends approval.)

h. Consideration of a request from the Elk Grove Park District, located at 499 Biesterfield Road, to waive FY 2017/18 business license fees in the amount of S3,000.

(It has been past practice of the Village Board to grant fee waivers for governmental and non-profit organizations. (The Director of Finance recommends approval.)

i. Consideration of a request from the Elk Grove Park District to waive permit fees in the amount of $738 for sidewalk and curb installation as part of the park improvements at Morton Park located at 957 Brantwood Ave.

(It has been past practice of the Village Board to grant fee waivers for governmental and non-profit organizations. (The Director of Community Development recommends approval.)

j. Consideration of a request from Golden Corridor Advanced Manufacturing Partnership for 2017 sponsorship dues in the amount of $10,000.

(GCAMP is the Golden Corridor Advanced Manufacturing Partnership. Their objective is to create awareness, training, and internship opportunities regarding advanced manufacturing jobs in order to develop a pipeline of qualified talent for manufacturers in the region. (Elk Grove Village, Hoffman Estates, and Schaumburg are municipal partners in GCAMP.)

k. Consideration to award a professional service contract to TranSystems Corp. of Schaumburg, IL for Phase II Engineering Services for the modernizing and extending to the west the existing lighting system at Devon Avenue and Tonne Road/Wood Dale Road in the amount of $30,648.28 from the BLF Fund.

(A proposal was Solicited from TranSystems Corporation of Schaumburg, IL for Phase II Engineering Services to develop contract plans and specifications for the modernization of the existing lighting at Devon Avenue and Tonne Road/Wood Dale Road intersection and extending the existing lighting to the west 1,000 feet along Devon Avenue. (TranSystems Corp. has successfully completed design and construction engineering for numerous Village projects. (It is recommended that a professional Phase II Engineering Service contract be executed with TranSystems Corporation for the Devon Avenue and Tonne Road Lighting Modernization project in the amount of $30,648.28. (Adequate funds are available in the BLF Fund. (The Director of Public Works recommends approval.)

l. Consideration to award a purchase contract to Currie Motors of Frankfort, IL for one (1) Ford Interceptor Utility in the amount of $32,289 from the General Fund.

(Police unit 241 was recently declared a total loss subsequent to a vehicle accident on March 19, 2017. (The Village received a settlement for the vehicle in the amount of $22,482.79. (This squad was less than three years old and had less than 50,000 miles (The Ford Police Interceptor Utility is available at a cost of $32,289 (Deducting the settlement amount from this cost leaves a net fiscal impact of $9,806.21) (The Chief of Police recommends approval)

m. Consideration to award a purchase contract in the amount of $39,083.51 through the National Joint Powers Alliance (NJPA) Joint Purchasing Cooperative to Casey Equipment of Arlington Heights, IL for the purchase of one (1) DynaPac CC1200 47" Drum Asphalt Roller from the Capital Replacement Fund.

(The Department of Public Works is purchasing one (1) DynaPac CC1200 47" Drum Asphalt Roller. (The DynaPac CC1200 47" Drum Asphalt Roller is available with base pricing established by the NJPA Joint Purchasing Cooperative and offered from Casey Equipment of Arlington Heights, IL. (The paver will be utilized for street paving and patching operations. (Adequate funds are budgeted in the Capital Replacement Fund. (The Director of Public Works recommends approval.)

n. Consideration to award a purchase contract to Currie Motors of Frankfort, IL in the amount of $70,080 for the purchase of two (2) Ford Pick-up Trucks with snowplows.

(In the FY18 budget, funds have been allocated to replace unit 740 in the Public Works Department Infrastructure Division and unit 825 in the Public Works Department Operations Division, both due to excessive age and mileage.

(The Ford F series pick-ups with plows are available through the Suburban Purchasing Cooperative (SPC) Joint Purchasing Contract from Currie Motors of Frankfort, Il (Sufficient funds are available in the General Fund and Water/Sewer Fund. (The Director of Public Works recommends approval)

o. Consideration to award a purchase contract to LDV Inc. of Burlington, WI in the amount of $219,011 for the purchase of a 2017 Ford F-550 with custom squad body.

(The Fire Department is replacing unit 140, a 1986 Ford F-8000, currently outfitted as a Water rescue Squad. (The Ford F-550 with custom squad body is available through GSA contract from LDV Inc. of Burlington, WI.

(Adequate funds are budgeted in the Capital Replacement Fund. (The Fire Chief recommends approval)

p. Consideration to award a contract to the lowest responsive and responsible bidder Landmark Contractors of Huntley, IL for the 2017 Crosswalk Rehabilitation Contract in the amount of $244,906.49 from the Residential Enhancement Fund.

(On Tuesday, June 6, 2017, the Village opened sealed bids for the 2017 Crosswalk Rehabilitation Contract. (Three (3) contractors obtained bid documents and three (3) submitted bids as shown on the attached bid tabulation of contractors' bid prices. (The lowest responsive and responsible bid was received from Landmark Contractors of Huntley, IL. (This contract provides for removal of brick crosswalk pavement at various locations across Village roadways and the replacement with new brick crosswalks. (Adequate funds are available in the Residential Enhancement Fund. (The Director of Public Works recommends approval.)

. Consideration to award a contract to the lowest responsive and responsible bidder R.W. qDunteman Co. of Addison, IL for the Leicester Road Rehabilitation Project (Biesterfield Road to Wellington Avenue) in the amount of $1,334,998.51 from the Water and Sewer, and Capital Improvement Funds.

(On Tuesday, June 6, 2017, the Village opened sealed bids for the Leicester Road Rehabilitation Project (Biesterfield Road to Wellington Avenue). (Ten (10) contractors obtained bid documents and six (6) submitted bids as shown on the attached bid tabulation of contractors' bid prices. (The lowest responsive and responsible bid was received from R.W. Dunteman Co. of Addison, IL. (The project includes full depth pavement removal and replacement, curb, sidewalk and driveway apron removal and replacement, new water main installation, repairs to the existing storm sewer system and landscaping restoration. (Adequate funds are available in the Water and Sewer, and Capital Improvement Funds. (The Director of Public Works recommends approval.)

r. Consideration to award a professional services contract to Nicholas & Associates Inc. of Mt. Prospect, IL for a total contract amount not to exceed $ 1,370,000 for Architectural & Engineering Services provided prior to construction and reimbursable non-fixed general expenses related to the design of the new James P. Petri Public Works Facility.

(This agenda item includes professional services associated with the design and planning of the new James P. Petri Public Works Facility. (Nicholas & Associates has been performing design and planning the new facility. (This item also includes reimbursable costs that Nicholas & Associates incur related to the planning and design Such as Surveying, environmental testing, Soil borings, permit fees to other regulatory agencies and other similar expenses. (The reimbursable costs are a direct pass through from Nicholas and are not marked-up with any profit or overhead. (This contract amount represents a portion of the construction management costs, after the bids are opened an additional contract will be prepared in accordance with the professional services contract with Nicholas & Associates approved at the February 28, 2017 Board meeting to act as Construction Manager and Constructor as a standard a project identification exhibit.

(The Director of Public Works recommends approval.)

s. Consideration of a request to hold a Public Hearing before the Plan Commission to consider the petition submitted by Maison Du Val for an Annexation and Resubdivision of the properties at 751 Meacham and 1050 Home Avenue.

(Maison Du Val is seeking an Annexation of the properties located at 751 Meacham Road and 1050 Home Avenue in unincorporated Cook County for the purpose of construction of six (6) new multi-family townhome buildings. (A date for the Public Hearing has not been established.)

t. Consideration to adopt Ordinance No. 3500 amending the Position Classification and Salary Plan of the Village of Elk Grove Village (Secretary-Police Department; Senior Clerk-Police Department; and Administrative Specialist-Police Department).

(The ordinance deletes a Police Department position of Secretary and deletes a Police Department position of Senior Clerk, and adds a Police Department position of Administrative Specialist.)

u. Consideration to adopt Ordinance No. 3501 amending Title 8, Section 8-3C-1-B of the Village Code, Amendments to the NFPA National Electrical Code of the Village of Elk Grove Village, by adding MC cable as a conditional permitted use.

(This Ordinance will amend our current code to limit the installation of MC (metal clad) cable. (This ordinance will provide limitations consistent with other similar types of cable installation methods. (The Director of Community Development recommends approval.)

v. Consideration to adopt Resolution No. 31-17 amending Resolution 36-98 and authorizing the Mayor and Village Clerk to execute a First Amendment to the Water Tower Lease Agreement between the Village of Elk Grove Village and New Cingular Wireless PCS, LLC at property located at 1141 Hawthorne.

(This amendment formalizes the obligation of New Cingular Wireless PCS, LLC to cover the costs of 3rd party inspections necessary to ensure compliance with engineering, aesthetic and other requirements as a result of equipment upgrades, installations, or other Site changes. (The amendment also eliminates the pending rent renegotiation clause, as the current monthly rent has been determined to be appropriately set at the market rate. (Additionally, the amendment clarifies that any equipment needed to meet local, state or federal requirements can be added to the Water Tower with no change to the monthly rent. (All other terms and conditions of the Ground Lease remain intact, including the annual 4% rent escalator.)

w. Consideration to adopt Resolution No. 32-17 authorizing the Mayor and Village Clerk to execute an Intergovernmental Agreement between the Village of Elk Grove Village and the Village of Arlington Heights for use of the Elk Grove Village Police Department Jail facility.

(Consideration to adopt an Intergovernmental Agreement with the Village of Arlington Heights and Elk Grove Village for use of the Elk Grove Village Police Department Jail facility during construction of the Arlington Heights Police Department building. (The Village of Arlington Heights has requested use of the Elk Grove Village Police Department Jail facility during construction of their new police building. (The Intergovernmental Agreement lists the duties and responsibilities of both parties. (The Police Chief recommends approval.)

x. Consideration to adopt Resolution No. 33-17 ascertaining the Prevailing Rate of Wages for Workmen on Public Works projects.

(The State of Illinois requires the Village to adopt this Resolution each year to determine the prevailing rate of wages for laborers performing construction of Public Works projects. (The rates are determined by the Illinois Department of Labor throughout Cook and DuPage Counties.)

y. Consideration to adopt Resolution No. 34-17 authorizing the Mayor and Village Clerk to execute a Real Estate Sale Contract between the Village of Elk Grove Village and The Berthold Land Trust Dated March 5, 2015.

(This property is being purchased for the purpose of constructing a new Public Works Administration and Operations facility. (The new facility will replace the Village's existing facility located at 600 Landmeier Road. (A copy of the Real Estate Contract will be available at the Village Board Meeting.)

7. REGULAR AGENDA

a. Consideration of a request from Elk Grove Bowl, 53 S. Arlington Heights Road, to allow a maximum of thirty (30) coin operated amusement device licenses per year.

(Elk Grove Bowl has requested approval to allow up to thirty (30) coin operated amusement devices within their facility. (Elk Grove Bowl plans to remodel their current gaming area to add additional coin operated amusement devices to attract and host more child friendly parties and events. (Section 3-3:B 2 of the Village Zoning Ordinance and section 3-4H-1 of the Municipal Code permit up to ten (10) coin operated amusement devices within a facility. (The Director of Community Development recommends approval.)

b. Consideration to approve sign variations for Brown Commercial Group Inc. located at 1400 E. Higgins Road.

(Variations are necessary to permit a monument sign which exceeds a height of six feet (6'), a width often feet (10) and thirty nine (39) square feet per side for a total sign area of seventy eight (78) square feet.

(The proposed sign will modify an existing sign in the same location. (The Director of Community Development recommends approval.)

a. PC Docket 17-2 - A Public Hearing for a Special Use Permit to allow the operation of an automotive repair center in an I-1 Industrial District for the property located at 160 Martin Lane. (PH 6-19-2017)

b. PC Docket 17-3 - A Public Hearing for a Special Use Permit to allow an Administration Building in an R-3 Single Family Residential District for the property located at 1001 Leicester Road. (PH 6-27-2017)

a. ZBA Docket 17-2 - Public Hearing to consider a petition for a variation from the provisions of Zoning Ordinance 2410 as it pertains to permitted locations for fences in residential zoning districts on property located at 216 Victoria Lane. (PH 5-25-2017)
